The flexibility and size of project teams meant that team-members became less familiar with the capability of their team mates.

Communication within a project team (including the customer) was increasingly more difficult. This was due to the complexity of projects, size and make-up of teams and the lack of familiarity within teams.

This led to a lot of friction between groups in the project because work items (such as requirements, designs, or implementation) were “thrown over the wall”.

This was exasperated by the reporting structure that had been put in place. Project managers, business analysts and consultants reported to one board member. Development and quality assurance staff to another.

The second restructure was an attempt to address this friction.
